Caring for Complex Patients

About the Program

The Center for Interprofessional Education and Collaborative Care is offering Caring For Complex Patients, a special learning experience for health sciences students in the Fall Spring semesters.

By participating in this experience, you contribute directly to improved health outcomes of patients in your own community; enhance your professional practice; and provide opportunities for personal scholarship.

With faculty support, your interprofessional student team will engage with two complex patients in a variety of settings (e.g.: home, community, primary care, or hospital). Student team may provide care coordination support, including: finding and connecting patient(s) to community resources, and providing a soft handoff back to the patient’s primary care at the end of the experience.
