Center for Healthy Hearts

About the Program

The Center for Healthy Hearts is a collaboration between faculty and students from the Schools of Pharmacy and Medicine within a free clinic setting. At the Center, care is led by pharmacists who managed the chronic diseases of indigent patients. Physicians support the pharmacists by evaluating complex patients through a referral mechanism. Students from both professions are engaged in this approach to care, exposing them to a new model of primary care that meets the needs of the underserved while also controlling healthcare costs. Patients enjoy the care received at the Center, and the measures for control of chronic disease exceed national benchmarks.
